GLPTN+LDITo apply for participation in the GLPTN+LDI initiative, please fill out this form. GLPTN will select 12 candidates based on their applications. Only one applicant per organization need apply.  Applications will be accepted no later than 5 p.m. on August 30, 2017.

 

 


 

GLPTN members of all sizes and medical specialties are eligible to apply for participation in the GLPTN+LDI initiative, but you must be based in Indiana.

Supported by coaching from Purdue Healthcare Advisors' lean experts and your organization’s assigned GLPTN Quality Improvement Advisor, candidates will launch their selected Lean Daily Improvement (LDI) project in October; work toward an LDI certification; attend several online training sessions; and complete the selected project in 3-1/2 months.

The first three project options are the core clinical measures GLPTN is committed to helping its clients improve. If you choose TCM or CCM enhancements, your organization should not be in the initial stages of implementing the code.

Once you submit this application, you will be included a pool of GLPTN members interested in participating in GLPTN+LDI. A representative from Purdue Healthcare Advisors will get back to you before September 1, 2017, regarding your interest.
