GLPTN+LEANThe Great Lakes Practice Transformation (GLPTN), in partnership with Purdue Healthcare Advisors' lean service team, is providing to select members a grant-subsidized package of lean services worth more than $12K. GLPTN+LEAN builds capacity for ongoing practice improvement and sets the stage for the evolution of the future practice environment. Implementation of this formal process improvement methodology may benefit all areas of an organization as well as satisfy components of MIPS, if applicable. GLPTN+LEAN A half dozen Indiana organizations are participating in the first round of the GLPTN+LEAN initiative. The second round is anticipated to begin in September 2017.

Once you submit this pre-engagement questionnaire, your organization will be included a pool of GLPTN-participating organizations interested in participating in GLPTN+LEAN. You also are encouraged to express your interest directly to your GLPTN Quality Improvement Advisor (QIA). A representative from Purdue Healthcare Advisors will get back to you before April 2017 regarding your interest.
