Clinical Quality Improvement Contractor

Enrollment Status: Pre-enrollment/fill out Expression of Interest Form below. Formal enrollment anticipated to begin in December 2019 and remain open through June 2020.

About the Program

Programmatic Aims

Earlier this year, the Centers for Medicare and Medicaid Services (CMS) issued a request for proposals to support community-based, primary care clinicians in quality improvement projects over a five-year period. The Clinical Quality Improvement Contractor (CQIC) program will focus on four areas:

Aim 1 ― Improve behavioral health outcomes, including a focus on decreased opioid misuse
Aim 2 ― Focus on patient safety and reducing all-cause harm including medication documentation, high-risk medications, dementia safety concerns, and antibiotic stewardship
Aim 3 ― Chronic disease management and prevention (cardiac and vascular health, diabetes, and kidney disease)
Aim 4 ― Improve community-based care transitions to reduce hospital admission

For questions about goals specific to the Indiana CQIC, email them to Natalie Stewart or Allison Bryan-Jungels.

Eligibility

The CQIC program is open to all primary care-oriented, Indiana-based healthcare organizations ― including rural hospitals, health clinics, community health centers, and FQHCs ― with an ambulatory setting of 10 or more clinicians. The CQIC program requires a commitment to one or more of the aims as well as to patient and family engagement. Participation also requires monthly data submission* on select quality improvement measures.

Benefits

  • Participate at no cost or risk to you
  • Optimize patient health outcomes, reduce variation and improve clinical care
  • Spend more time caring for your patients
  • Improve targeted quality measures (alignment with value-based contract measures/Quality Payment Program) with direct assistance from a dedicated Quality Improvement Advisor
  • Spread and sustain evidence-based, best practices throughout your organization with Lean Daily Improvement (LDI) training, coaching, and certification
  • Collaborate with clinician colleagues locally, regionally, and nationally to accelerate innovative care strategies

*Monthly data for all applicable clinical quality measures (CQMs) in a standard numerator/denominator format. No patient-level data requested. Blind submission to CMS.

Expression of Interest Form

If you are interested in participating in the CQIC program, please fill out the form below and we will contact you when enrollment begins. This is a non-binding intent to participate form.

We assume you are inquiring for an Indiana-based organization.

If you are a hospital, just provide the numbers for the ambulatory side.

This answer is important because some EHRs may not be able to produce the data needed for this initiative.

Next Steps

Once you submit this form, your organization will be included on our list of organizations interested in participating in the CQIC program. Once enrollment begins, a representative from Purdue Healthcare Advisors will contact you.

Funding

The award is anticipated in late 2019 from the Centers for Medicare and Medicaid Services (CMS).
