Determining Eligibility
To apply for participation in the GLPTN+LEAN initiative, an organization must be a GLPTN-participating member. If you are not sure whether your organization is a GLPTN member, just say so and we'll check for you.

Eligibility to participate in GLPTN+LEAN Round II requires that you be new to the initiative.

GLPTN+LEAN Round II includes an on-site strategy session; LDI training; and two RIEs. For programmatic success, it is vital that you have executive leadership support; an internal passion for quality improvement with a commitment to sustainability of the new workflow; and provide sufficient time and resources to complete each project.

GLPTN+LEAN Round II eligibility requires 15+ clinicians.
More About Your Organization
The two GLPTN+LEAN Round II PHA-facilitated Rapid Improvement Events (RIEs) will align with GLPTN's aims: reduce unplanned, unnecessary readmissions (i.e. Transitional Care Management/TCM); improve the referral process; improve health outcomes through Depression Screening; Diabetes Control (A1C); and Hypertension Control; reduce unnecessary testing; reduce unnecessary hospitalizations; and expand access. Your answer does not serve as a commitment by you to focus improvement on any of the areas you mention. This is just a quick brainstorm!

Once you submit this pre-engagement questionnaire, your organization will be included a pool of GLPTN-participating organizations interested in participating in GLPTN+LEAN Round II. You also are encouraged to express your interest directly to your GLPTN Quality Improvement Advisor (QIA). A representative from Purdue Healthcare Advisors will get back to you by mid-January 2018 regarding your interest.
