LHTI Year 4 Registration

Year 4 of the Lean Healthcare Transformation Initiative begins with your decision to stand up a Lean Office in your organization, the first requirement for Lean Office Certification. Please review the requested information for Year 4: Certification of Lean Offices and then have all information ready ahead of time to cut-and-paste it into this form. This form cannot be saved.

Candidate Information

Each Lean Office must have at least one Executive Lean Leader leading it, and may have a maximum of two. Executive Lean Leaders will lead the Enterprise Strategy Session for their organization in February (dates listed later in this form). List your Executive Lean Leader Candidate(s), providing name, job title, email address, and lean experience/certification level achieved. Experience may include Lean Certification(s), Process Owner, Value Stream Leader, none, etc.

Note: All Executive Lean Leader Candidates without a lean tool certification must be registered to complete Lean Daily Improvement Training and Certification this year to meet the Executive Lean Leader Certification requirements. You will have an opportunity to register them for this training later in this form.

Executive Lean Leader Candidates

Each organization has the opportunity to train up to five (5) Lean Leaders including the Executive Lean Leader(s). If you have one (1) Executive Lean Leader Candidate, list up to four (4) Lean Leader Candidates; if you have two (2) Executive Lean Leader Candidates, list up to three (3) Lean Leader Candidates.

Note: All Lean Leader Candidates without a lean tool certification must be registered to complete Lean Daily Improvement Training and Certification this year to meet the Lean Leader Certification requirements. You will have an opportunity to register them for this training later in this form.

Required Activities/Training for Year 4

Choose from the following dates/times for your group (Lean Leader and Executive Lean Leader candidates) to attend a 1-1/2 hour, interactive webinar that introduces Year 4.

Choose your preferred date for a one-day (8 hours) Lean Leader Training session.

Enterprise Strategy Session

During the month of February, each organization will execute an Executive Lean Leader-led Enterprise Strategy Session, which your PHA Coach will attend at your home location. Each session will be scheduled by PHA according to the following schedule to assure support from our office:

  • February 5 – Cameron Memorial Community Hospital
  • February 7 – Franciscan Health Rensselaer
  • February 14 – Rush Memorial Hospital
  • February 19 – Harrison County Hospital
  • February 21 - Putnam County Hospital
  • February 26 – St. Vincent Randolph and St. Vincent Randolph Mercy Hospitals
  • February 28 – Fayette Regional Health System

Lean Daily Improvement (LDI) Training and Certification

LDI Training and Certification is required for Lean Leader and Executive Lean Leader Candidates who have not earned a lean certification. More about LDI Training and Certification can be found here. Training will be held remotely on one day each week during the month of March, or training can be completed on-line at the convenience of the candidate.

Note: In order for an organization to be designated as a Certified Lean Office, its Executive Lean Leader Candidate(s) must have all their applied work required for LDI Certification completed by June 1st.

Indicate the 1) Remote Webex Series or 2) On-line Lean Daily Improvement next to each candidate's name.

Additional Skill-Building Opportunities (Optional)

Lean Practitioner (LP) Training will be awarded for up to six (6) candidates based on whether an organization needs this capability within its Lean Office. Training will be held January 28-31, 2019, on the Purdue University West Lafayette Campus unless an organization volunteers to host it. More about LP Training can be found here.

An LP plans, delivers and facilitates-to-closure Kaizen and Rapid Improvement Events.

Thank you for taking the time to register for the LHTI's Year 4: Certification of Lean Offices. PHA will send verification to all registrants and "cc" the main contact at each site by November 30th. Upon submission of this form, your main contact will receive an email confirming registration and listing your response.
