Quality Payment Program Resource Center (QPP-RC)

A multi-year program that provides approximately 2,900 Indiana clinicians with remote services ― including educational webinars, a help desk and a resource library ― to improve their Merit-based Incentive Payment System (MIPS) metrics.

Indiana Medicaid Promoting Interoperability Program

A program that provides remote and on-site services ― including direct technical assistance, an annual security risk assessment as well as other health IT-optimizing services ― to up to 175 Indiana-based, ambulatory organizations that are eligible to participate in the program due to the number of Medicaid encounters they bill.

Lean Healthcare Transformation Initiative

A program that works with Indiana critical access hospitals, rural hospitals, and behavioral health organizations to build internal capacity for improvement management by training their staff to effectively deploy improvement events, analyze value streams, and solve complex challenges.

Small-Care Neighborhood ― Opioids Response

A program to improve the quality and cost of care for Indiana Medicaid by helping two communities respond in a strategic, community-focused way to the opioids epidemic.

Small Care Neighborhood ― Long Term Care

A program to improve the quality and cost of care for Indiana Medicaid by helping two communities to improve care transitions, medication management, and processes surrounding long term care.

Empowered Communities Initiative (ECI)

A program to help an Indiana community reduce prescription opioids use and increase patient access to addiction treatment and recovery services.

Clinical Quality Improvement Contractor (CQIC)

A five-year program to support community-based, primary care clinicians in quality improvement projects for improved behavioral health outcomes; patient safety; improved chronic disease management; and better transitions to reduce unnecessary hospital readmissions.

Health Systems Change Partnerships

A multi-year state initiative to create systems of intervention inside Indiana hospitals to help people quit smoking. 

Quality Processes for Health Improvement

A multi-year initiative to re-engineer Marion General Hospital’s chronic disease management of patients diagnosed with heart failure.
